They are replaced with the caption suggesting that one must refer to the Printed Edition. In this day and age, this kind of thing is unacceptable. Either have the WHOLE book, or none at all. This is simply not a good idea! This devalues the product, and I would not recommend||This valued text belongs on the reference shelves in the libraries of our colleges of medicine and nursing, as this text could serve as the primary reference for an entire semester ethics course.--Journal of the National Medical Association||
